New Patient Registration-

To register at this practice we require TWO forms of identification and either a medical card or registration form (GMS1).

These may include:

• Driving Licence or passport

• Utility Bill (water, gas, electric, etc.)

• Bank statement or council tax statement

• Tenancy agreement

• A letter from your employer

If you have recently just arrived in the UK from abroad and have not had a GP in this country before, we do require evidence of status i.e. Visa/residence permit/work permit or passport and one of the above.

 

How to register with a medical card

• Amend any address data

• Date and sign the card

 

How to register without a medical card

You will be given a GMS 1 form to complete. 

Essential information we require:

• Name and address of your last doctor

• The address at which you were registered with that doctor

• If you are from overseas, your place of birth and the date you arrived in the UK

• Your signature

 

To Complete Your Registration

You will also be given a health questionnaire to fill in.

Please hand all forms into reception between 13:00 – 17:00 (except Wednesday).

You may say which doctor you prefer to be registered with. If you wish to have new patient check the please make an appointment to see the practice nurse.

Once we have accepted you onto our list you will be registered with the Practice and may consult any doctor. However, for both new and existing patients we suggest it is best to try to remain with one doctor for any one illness, and we believe it is very much to your advantage if you regard one of the doctors as your regular doctor, especially if you suffer from a chronic illness or are on long-term medication. It may not always be possible to see your regular doctor due to absences from the Practice.
